Yes, anything that uses an offset shank is going to be trouble.
Each car needs to be evaluated on it's own. The biggest factor being, can you find a way to mount the coupler up against the bottom of the car without a typical coupler box.
The Athearn 89' flat used for TOFC and both the bi-level and tri-level racks is actually pretty easy to convert with an adapter.
