Is it possible to go from one block controlled by one DCC system to another block controlled by another system?

Yes and no.
Once you get to the other system the other command station would somehow have to automatically acquire the address to continue the running.

You'd have to have the given address set to the same speed step on both systems. Also, I would have some concerns about bridging the gap, should the two system outputs be out-of-phase (which is almost a certainty).
